Dental Hygiene Program Deadlines
February 1
All applications for admission into the Dental Hygiene Program must be received by the IPFW Dental Hygiene Department on or before February 1 each year. Late applications will not be accepted. Submit the following information to the IPFW Dental Hygiene Program by February 1 of the year in which you are applying for enrollment:

IPFW Dental Hygiene Program Application Applications are received by contacting the Dental Hygiene Program and requesting a Dental Hygiene Program Applicant Packet of information.
NOTE: Copies of applications from other programs are NOT accepted.
- IPFW Curriculum Information Form Contained in the Applicant Packet.
- Official college transcripts from all colleges/universities attended*
*Special note: IU, Purdue, and IPFW students can submit unofficial transcripts.
- Observation Verification Form Contained in the Applicant Packet. Completion of a minimum of two - two hour observations in a dental office, observing a dental hygienist providing patient care services is required for admission into the IPFW Dental Hygiene Program. Dental Hygiene Observation Verification Forms are also available on the website.
June 1
All prerequisite courses must be completed with a grade of C or better by June 1 for fall admission into the program. No exceptions are given for completion of summer courses. Final transcripts must be received by the Dental Hygiene Program no later than May 25 unless the course ends after May 20, but before June 1.
